Why Does the Drive Fault to 'Speed Feedback'?

The tachometer or encoder may be bad. Switch the drive speed feedback to armature voltage, and then verify normal operation.

Open up DSELite and monitor the feedback, is it good feedback?

Try swapping the option card, the cables, and the motor if at all possible to see if the issue follows one of those devices.
